Watch the process being done before writing about it.

Documentation method:

1. Shadow someone doing the task
2. Record video or take detailed notes
3. Ask questions: "Why this step? What if X happens?"
4. Note variations between people
5. Identify decision points and exceptions

Do not assume. Observe reality, not what people think they do.
